NU imPACKt Points Submission Form

By completing this form, you are requesting that your listed activity be added to your NU imPACKt activity for the badge series you identify. Submissions must be completed within 10 business days of the event or activity.

To initiate the process for adding points to your imPACKt activity record, please submit the form below. Leadership and Go MAD submissions will be forwarded to the advisor, coordinator, or sponsor of the activity, who must complete confirmation to validate your involvement.

Some events will have a QR code to scan in and out of the event. You will still need to complete the form requesting your points toward a badge be added to your record. For the advisor, coordinator, or sponsor information, enter the following:
