I hereby authorize the release of my student academic information. This information will be used to make decisions on proposed membership and other administrative purposes. The information is limited to current (cumulative and semester) GPA, and for first year students, High School GPA. This information is also used by the university to determine Fraternity & Sorority academic rankings for national offices, shared with an individual’s chapter or national office upon request, and provided to campus departments. The university reserves the right to forward student information to campus departments for non-profit use. Also, I give Northwood University permission to photograph me. Northwood University has the permission to use these photographs/videotapes for publicity purposes, unless written notice is received to the contrary.
Greek Students must have a 2.3 cumulative GPA to start the new member process as well as remain active in the Greek organization on behalf of Northwood University.

Hazing: The university prohibits any student, acting alone or with others, to conspire to engage in hazing to participate in hazing. Hazing is defined as any action taken or situation created, whether on or off campus, for the purpose of initiation or affiliation with any organization or team which jeopardizes the physical and/or mental well-being of any individual, or is, presumably, an illegal activity.
Freedom from humility and danger of hazing is guaranteed to every student on this campus. This policy includes individuals as well as entire chapters. Any violation of this policy should be reported immediately to the Office of Student Life, in which the situation will be referred to the appropriate judicial body and appropriate action will be taken.
As a fraternity or sorority new member, I pledge that I will work to eliminate all form of hazing practices found within my chapter as well as to uphold all other rules and regulations of Northwood University.
